Meniscal root repair
Meniscal root repair is a specialised arthroscopic procedure performed by Dr Damasena to restore the function of the meniscus when it has torn at its attachment (root) to the bone. These injuries are important because a root tear effectively renders the meniscus non-functional, similar to having it removed, which can lead to increased joint stress and early arthritis. Repair is typically recommended in patients with symptomatic root tears, particularly those with relatively preserved cartilage, minimal arthritis, and suitable limb alignment. It is most commonly performed in younger or active patients, but may also be appropriate in selected older individuals depending on joint health.
The procedure is performed using keyhole (arthroscopic) techniques. Dr Damasena repositions the torn meniscal root back to its native attachment site on the tibia. This is usually achieved using sutures passed through the meniscus, which are then secured through small tunnels created in the bone (a transtibial pull-out repair). This restores the hoop stresses of the meniscus and helps it function as a shock absorber again. In some cases, additional procedures such as ligament reconstruction or alignment correction may be performed at the same time if required.
Recovery from meniscal root repair is more protective than standard meniscal surgery, as the repair needs time to heal securely to bone. Patients are typically required to use crutches and limit weight-bearing for approximately 4–6 weeks. A brace may be used to restrict knee flexion, particularly in the early phase, to reduce stress on the repair. Swelling control, gentle range of motion exercises, and quadriceps activation begin early under the guidance of a physiotherapist.
Post-operative rehabilitation is structured and progresses in phases. In the first 6 weeks, the focus is on protecting the repair while maintaining knee extension and gradually increasing flexion within safe limits. From 6 to 12 weeks, weight-bearing is progressively increased, and strengthening exercises are introduced. Higher-impact activities, running, and pivoting sports are typically delayed for several months, often around 4–6 months or longer depending on healing and individual progress. Dr Damasena and the physiotherapy team will closely monitor recovery to ensure a safe return to activity while optimising long-term knee function.
